Absences, Tardies and Early Dismissals

The booklet is provided for all all Absences, Tardies and Early Dismissals. Please print out this form and either fax it to the office at 941-404-4492, or send it in with your student. If you have any questions, please contact Kathy Bechtel at 941-377-2278.

Change of Address Form

If making a change of address, please fill out the attached form and you must attach proof of address: utility bill or lease agreement in the parent or guardians’ name.  If you do not have any bills in your name, please contact the Registrar for further direction.  ​

Required Vaccinations:

Students must have required vaccinations before school starts August 12, 2019
